Assessment of Drug Candidates’ Cardiac QT Liability
Before new drug candidates can be tested in humans, it is critical to assess whether they will provoke an irregular heart rhythm. The U.S. Food and Drug Administration requires preclinical testing on all drug candidates to ensure they don't cause QT interval prolongation on an electrocardiogram: an abnormality of the heart's electrical system that can lead to a potentially life-threatening arrhythmia and cause sudden death.
Gan-Xin Yan, MD, PhD, has pioneered a method using slices of heart tissue from the adult ventricle of a rabbit that delivers quick, accurate testing. The invention, known as arterially perfused ventricular wedge preparation (wedge prep, for short), records essential electrical and mechanical signals in the heart that can be used to accurately identify drug-induced changes in any of the important ionic channels involved in ventricular depolarization and repolarization. The wedge prep marks a pivotal advance as scientists using an in vitro preparation previously could record only the signals each time of one ionic channel. The electrical stability of the wedge prep lasts much longer than that of any other preparation, a critical requirement for assessing the QT liability of drug candidates.
